The Registrar’s Office provides unofficial and official copies of transcripts. Please note any release of an academic record requires the student's signature.

To order an official transcript, please utilize the National Student Clearinghouse secure site. Please be mindful of the following items when ordering an official transcript:

  • Official copies are available in either PDF or paper form. 
  • Each copy costs $10 and can be ordered using any major credit/debit card. 
  • Please allow 2-3 business days for processing, plus any US mailing time.
  • An official copy includes the seal, the Registrar’s signature, and is printed on security paper.
  • Transcripts are only released after all outstanding business is completed with the Business Office and the Financial Aid Office.

To order an unofficial transcript, please fill out this form (pdf) and either drop it off at our office, or send via email or fax. 

  • Unofficial copies are free of charge.
  • Unofficial copies do not have a long processing time, and can be picked up immediately after the order has been processed.
  • Unofficial copies are printed on plain white paper as unofficial copies are often used for personal records.

Walk-in requests can be made at 211 Warde Hall during normal office hours. (Monday–Thursday 8 a.m.–5 p.m.; Friday 8 a.m.–4:30 p.m.; Summer: Monday–Friday 8 a.m.–4 p.m.)

  • Graduate Student Change of Emphasis (pdf)
    Use this form to change your emphasis.
  • Change of Major/Minor/Adviser (pdf)
    Use this form to add or drop majors, minors, and endorsements.
  • Change of Personal Information (pdf)
    Use this form in event of a legal name change, correction of a birthdate, change of gender, or updated social security number or individual tax payer I.D. number.
  • Enrollment Verification (pdf)
    Use this form to request an enrollment verification to be sent to another organization.
  • Independent/Directed Study (pdf)
    Use this form to register for an independent or directed study course. See catalog for more details.
  • Internship Registration (pdf)
    Use this form to register for an internship.
  • Pass/Fail (pdf)
    Use this form to declare to take an elective course pass/fail. See catalog for more details.
  • Petition (pdf)
    This is a multi-purpose form and is used in several scenarios. Please check with the Registrar’s Office with questions.
  • Request Replacement Diploma (pdf)
    Need a copy of your diploma? Complete this form and return to the Registrar’s Office with payment.

Teacher Education Licensure

Students need to submit to the Registrar's Office the application for teacher licensure. Please fill out the appropriate sections of the form and submit approximately four weeks prior to graduation. The application is available in the Education Division Office or in the Registrar's Office.

Mount Mercy students that wish to add endorsements and are already holders of valid Iowa teaching licenses will need to use the above mentioned form to add the endorsement to the license. Fill out the form, attach a check for the appropriate fee, and submit them to the Registrar's Office upon completion of all course requirements for adding the endorsement.
